About the Author

Integrative Physiotherapist · 20+ years in clinical practice

Caroline
da Silva

I have been a musculoskeletal physiotherapist for over twenty years. In that time I have conducted thousands of clinical assessments, and the conversation I have more than any other is some version of this one: a woman who knows something is changing, who has been seeking help, and who has been given information that, while not untrue, is so incomplete it is almost useless.

What that woman needs is not another supplement recommendation or a generic exercise plan. She needs to understand what is actually happening in her body — the physiology, the evidence, and the clinical reasoning behind the most important things she can do for her health from midlife onward.

This book is what twenty years of practice has taught me.

The Strong Her Pillars

From the Clinic

What twenty years of practice
actually teaches you

On muscle

"What the research on myokines has changed for me clinically is how I explain the purpose of strength training. Every time you challenge your muscles, you are initiating a biological conversation that influences your brain, your bones, your metabolic health, and your immune function simultaneously. The question is not 'how do I look better?' It is 'what am I building?'"

On nutrition

"The most surprising nutritional pattern I observe in women who are doing almost everything else right is that they consistently under-eat protein — particularly at breakfast. The muscle they are trying to build cannot be synthesised from a coffee and a piece of toast. Protein is not optional. It is the primary building material of the body that everything else in this book is asking you to invest in."

On consistency

"The women who age exceptionally well are not the ones who found the optimal programme. They are the ones who built a small number of good habits and kept returning to them — through busy seasons and difficult ones, through illness and recovery, through the years when everything felt harder. Consistency is not a personality trait. It is a strategy."

What's Inside

Fourteen chapters. One complete framework.

Each chapter builds on the last — from foundational biology to practical application — so that by the end, you don't just know more. You see your body differently.

1

Why Women Were Never Taught How Their Bodies Work

The research gap, the owner's manual you were never given, and why understanding is the first intervention.

5

Muscle: Your Longevity Organ

The science of myokines, anabolic resistance, and why muscle is the most powerful health investment available to women after forty.

9

Your Gut, Inflammation and Healthy Ageing

The microbiome, inflammaging, the estrobolome, and what a genuinely anti-inflammatory lifestyle actually looks like.

13

Balance, Mobility and Falls Prevention

Power vs strength, Tai Chi evidence, proprioception, and why fear of falling is often the greater disability.

2

Hormones: Your Body's Communication System

What oestrogen, progesterone, testosterone, insulin, cortisol and thyroid hormones actually do — and how they work together.

6

Strength Training: How to Actually Do It

Progressive overload, RPE, training during high-symptom periods, and how to structure a week that produces real adaptation.

10

Sleep: Your Most Powerful Recovery Tool

Sleep architecture, OSA in women, CBT-I, circadian rhythm, and why sleep deprivation undermines everything else.

14

The Strong Her Blueprint

The Six Pillars integrated into a practical framework for building a life in which health is the default, not the exception.

"Women do not need to spend the second half of life becoming smaller. They need to become stronger, more capable, and more confident in their bodies."

3

Perimenopause: The Transition No One Prepared You For

Why it starts earlier than most women expect, what hormonal variability means for symptoms, and how to navigate it.

7

Building Strong Bones: Protecting Your Skeleton for Life

Bone remodelling, the four pillars of bone health, collagen, calcium, vitamin D, and when to seek a DEXA scan.

11

Stress, the Nervous System and Cortisol

The HPA axis, the mental load as a clinical factor, autonomic balance, and what actually supports resilience.

4

Menopause: A New Beginning, Not the End

What changes across body systems, the current evidence on MHT, and how to prepare for the decades that follow.

8

Food as Fuel: Nourishing Your Body Through Midlife

Protein targets, carbohydrates, healthy fats, fibre, creatine, alcohol, and an honest assessment of intermittent fasting.

12

Cardio for Women: Training Your Heart for Life

VO₂ max, Zone 2 training, cardiovascular risk after menopause, and why walking is more powerful than you think.

The Strong Her Framework

Six Pillars. One integrated system.

Everything in this book is built around six foundations. They are not separate topics — they are a connected system, and improving any one tends to support the others.

1

STRENGTH

Build it. Maintain it. Never stop. Muscle is the longevity organ — the most powerful investment a woman can make in the second half of her life.

4

RECOVERY

Adaptation happens during recovery, not during training. Sleep, rest, and deliberate restoration are not optional — they are where progress is made.

2

MOVEMENT

Your body was designed to be used daily — not just during scheduled exercise. Movement throughout the day supports everything else in this book.

5

CURIOSITY

A learning mind is a healthy mind. Cognitive challenge, creative engagement, and lifelong learning support the brain's long-term health and resilience.

3

NOURISHMENT

Food is information, not punishment. Adequate protein, varied whole foods, and consistent hydration are the nutritional foundations of strength.

6

CONNECTION

The most underrated health behaviour. Meaningful relationships are consistently among the strongest predictors of long-term health and longevity.

WHO IS THIS BOOK FOR

For the woman who wants to understand, not just be told.

You are in your late thirties, forties, or fifties and something feels different — but no one has given you a satisfying explanation

You want to understand the science behind what is happening in your body — not just be handed a protocol

You are tired of conflicting wellness advice and want information from someone with genuine clinical expertise

You have been told your blood tests are 'normal' but you know your body well enough to know something has changed

You are already exercising and eating reasonably well, but you are not sure if you are doing the right things for this stage of life

You want to feel stronger, more capable, and more confident in your body — not smaller, lighter, or younger-looking

FROM CLINICAL PRACTICE ・CAROLINE DA SILVA

"What I have learnt after assessing thousands of women is that the most consistent predictor of how well a woman navigates midlife is not her genetics, her starting fitness level, or her access to resources. It is whether she understands what is happening in her body. Women who have that framework make better decisions, sustain healthier habits for longer, and experience less fear. The goal of everything in this book is that framework."
